Stiffness of knee after total joint replacement
Stiffness after total knee replacement is a very difficult and complex problem. It is the most common reason patients are unsatisfied with their total knee replacement. Stiffness, also known as arthrofibrosis, occurs when a knee replacement is not moved early enough through a wide range of motion. Scar tissue then begins to bind up the ligaments and soft structures surrounding the knee resulting in severe stiffness. Sadly, this stiffness is almost universally associated with significant pain. Limitation of activities such as the ability to ascend and descend stairs, get in and out of a car, and even ride comfortably in a car for prolonged periods of time are common complaints.
Treatment Options
Diagnosis is made with routine x-rays and physical examination. Treatment options for this condition are limited. Manipulation of the knee joint can be undertaken in the acute postoperative phase no later than 8 weeks after surgery. This has about a 50% success rate. After the scar tissue has matured however, conservative treatment and manipulation are ineffective for improving range of motion.
Surgical interventions when recommended, generally consist of a revision of the knee replacement with removal of components to allow for full removal of scar tissue with new implants being placed afterward.
Painful retained hardware of the knee
Fracture care and ligament reconstructions often require placement of instrumentation such as plates, screws, and rods. At times these surgical devices or hardware are in places where they can be felt through your skin. Commonly, we don’t recommend removing orthopedic hardware or other instrumentation used for surgical fixation of fractures unless it becomes painful. However, when prominent screws, plates, or other devices become bothersome removing them may be recommended.
Failed Total Knee Replacement (arthroplasty)
Rarely after total knee replacement, the total joint needs to be repeated or revised. Common reasons for this are infection, joint instability, component loosening, and severe stiffness.
Treatment Options
Evaluation for this begins with a thorough examination with x-rays and laboratory studies. depending on these findings, a revision total knee replacement may be recommended.
As with most repeat surgeries, revision knee replacement has higher risk of complication and difficulty in recovery. Therefore, great care should be taken to ensure good patient selection and rationale for revision surgery

Dr. Sanders, from Cascade Orthopedic Specialists, did a much needed right kbee replacement for me in October 31, 2024. I received top level care from their staff, both pre-op and post-op. Hospital care and treatment at Mercy was excellent. Rehab at Theropy Plus was painful for the first two weeks, but healing came quickly.
