Hip pain

Hip pain is a common and potentially limiting condition.  The hip connects your pelvis/spine to your legs. Several large and powerful muscle groups attach around the hip joint.  The hip joint is a ball and socket joint which consists of the top end of your femur bone (femoral head) and the hip socket (acetabulum) is located on the side of your pelvis. Pain can come from conditions related to this joint, its surrounding muscles, and spinal conditions. 

Treatment Options

Evaluating hip pain involves x-rays, a physical exam, and in some cases blood tests or advanced imaging such as MRI scans.   

Avascular necrosis of the hip

Several bones in the body are extensively covered in cartilage resulting in limited blood flow to these areas.  These bones include the ball of the hip, the ball of the shoulder, and specific wrist and ankle bones.  When blood supply to these areas is lost, a portion of the bone will die. When bones die, the involved area of bone may soften and eventually collapse resulting in pain, arthritis, and difficulty with ambulation.  The severity and progression of this disease depends on the size of the involved area and determines treatment options.  

Risk factors for avascular necrosis include alcoholism, high dose steroid use, hip dislocation, certain medications, hip fracture, and commonly no known cause is identified (idiopathic).  Evaluation for this includes a physical exam and x-rays. If avascular necrosis is suspected, an MRI confirms the diagnosis.

Treatment Options

If caught early, the area of affected bone may be removed and bone graft placed.  If collapse and arthritis has already occurred, total hip replacement may be the surgical treatment of choice.

Hip Bursitis or Trochanteric Bursitis

Bursa sacs are common structures found throughout the body in areas of friction where structures must slide over each other. In the hip, the largest bursa is found over the outside of the hip under a large thick band called the iliotibial (IT) band.  When this bursa becomes inflamed, it causes significant lateral or side hip pain.

Symptoms

Common symptoms include pain when lying on the affected side while sleeping and pain on the outside of the hip while walking.

Treatment Options

Treatment for this is largely conservative with anti-inflammatory medications, steroid injections, and physical therapy. Physical therapy is often used to help stretch the IT band and take stress off the bursa.  Rarely, surgical removal of the bursa sac is performed to alleviate pain. 

Iliotibial band pain

The iliotibial band is a long structure extending from the top of the pelvis to the outside of the knee just below the knee joint. This band can become tight and painful resulting in pain that can radiate from the hip all the way down to the knee. Sometimes the presentation of this is closer to the knee joint.

Treatment Options

Treatment for this focuses on physical therapy for stretching of the band along with anti-inflammatory modalities and medications. 

Tendon tears of the hip

The hip has several large muscle groups that attach to it and are critical for its function. Some of the most important muscle groups include the gluteal tendons which attach on the outside upper end of the femur. In rare cases, hip bursitis can also include gluteal tendonitis with tears. The spectrum of these tears can be mild too severe. 

Diagnosis of these tears usually is confirmed with an MRI scan. Oftentimes patients have been treated many times for hip bursitis without improvement or resolution.

Treatment Options

Treatment for gluteal tendon tears include anti-inflammatory medications, cortisone injections, and physical therapy. In cases where tears are painful and limiting, surgery is recommended when non-surgical treatments have failed.

Painful retained hardware of hip

Fracture care and ligament reconstructions often require placement of instrumentation such as plates, screws, and rods. At times these surgical devices or hardware are located in places where they can be felt through your skin. Commonly, we don’t recommend removing orthopedic hardware or other instrumentation used for surgical fixation of fractures unless it becomes painful. However, when prominent screws, plates, or other devices become bothersome removing them may be recommended.  

Dislocation of the Prosthetic Hip

An uncommon complication after hip replacement can be dislocation of the prosthetic ball and socket. This complication can occur within a few weeks of the surgery, or many years after surgery. Dislocation can be extremely painful and can cause inability to perform daily activities.

Treatment Options

There are various causes for prosthetic hip dislocation. These may include trauma, poor placement of prosthetic components, or severe wear of the prosthetic hip components. If dislocations occur multiple times, surgical revision is commonly recommended for the hip replacement.
