Iliotibial band pain
The iliotibial band is a long structure extending from the top of the pelvis laterally to the outside of the knee just below the knee joint. This band can become tight and painful resulting in pain that can radiate from the hip all the way down to the knee. Sometimes the presentation of this is closer to the knee joint.
Treatment Options
Treatment for this focuses on stretching of the band along with anti-inflammatory modalities and medications.
Kneecap subluxation
The kneecap is also known as your patella. It is a bone embedded in the quadriceps mechanism. The thigh muscles create a large tendon starting above your knee and extend down below it to allow for extension of the knee joint. The kneecap is embedded in this large tendon structure. It glides or articulates in a groove on the front of the femur. In some patients, the patella does not track well in that groove. When the patella is tilted in that groove it is called subluxation. In rare cases patients can even dislocate their kneecap because of this tilted position.
Treatment Options
Treating patellar subluxation can be difficult. Most often conservative treatments are recommended which include bracing and therapy. The various causes of this subluxation can be evaluated using CT scan and MRI. In some instances surgery is recommended to correct this poor tracking of the patella.

Loose bodies of the knee
Arthritis is a disease which occurs when a joint has lost or is losing cartilage. Cartilage is the soft rubbery padding inside all moving joints of the body. When the cartilage wears away, joint pain and stiffness result. As the cartilage deteriorates occasionally a large piece of it will fall off at one time. Since cartilage does not have blood supply, but lives off of joint fluid, these cartilage fragments can go on living indefinitely. Occasionally they grow and even calcify. This is known as synovial chondromatosis. When this occurs, these fragments can occasionally get stuck in the joint resulting in pain and locking symptoms.
These fragments can also occur in this setting of trauma when a piece of bone and or cartilage is sheared off of the joint.
Treatment Options
Treatment for loose bodies of the knee typically involve surgical excision. This can usually be done arthroscopically. In cases where an acute injury has occurred, the cartilage fragment may be replaced.
Internal cartilage damage (osteochondral lesion) knee
Cartilage is the soft rubbery padding inside all moving joints of the body. Occasionally due to trauma, cartilage can be damaged inside of the knee. Most often this happens on the end of the femur bone. This creates a cartilage defect or pothole on the surface of the joint. This can be very painful and also can lead to damage and deterioration over time in that area.
Treatment Options
This diagnosis is different than arthritis because it is an isolated type of injury in the setting of relatively normal cartilage and joint health. Diagnosis of this is usually confirmed with an MRI scan after routine X-ray and physical exam. Occasionally, these defects include a chunk of bone as well which can be visualized on X-ray. Treatment for this typically requires surgery. Surgical options depend on the size of the defect but may include placement of cadaver cartilage graft, or using the patient’s own cartilage and transplanting it to the defect.

Dr. Bishop and his team got me in last minute after my referral got lost between my PCP and their office. The front desk was polite and helpful. Their paperwork was minimal. X rays were quick and waiting time in the room was minimal before Dr. Bishop came in. He explained everything clearly and have me a copy of all of his recommendations before asking me to schedule a follow-up visit.

Dr. Sanders has been seeing me regarding my knee for the past 4 years. After all the years of dealing with knee pain and injection shots, it has been agreed that it is time to get a knee replacement. Dr. Sanders and his staff are incredible.

My little guy got the best care here after breaking his arm on the trampoline. We seen Benjamin Baird, PA, and were impressed with his genuine interest and kid approved humor. We hope we aren’t back anytime soon, but if there are any more breaks, we’d choose Cascade again.

Dr. Sanders, from Cascade Orthopedic Specialists, did a much needed right kbee replacement for me in October 31, 2024. I received top level care from their staff, both pre-op and post-op. Hospital care and treatment at Mercy was excellent. Rehab at Theropy Plus was painful for the first two weeks, but healing came quickly. I now have great range of motion and living pain free with my new knee. Thank you, everyone.

For my plantar fasciitis.From the scheduling of appointments, front desk help Amanda and my initial appointment w Dr. Bishop hands down the best experience ever. Had my 2nd appointment for cortisone shot, from the stories you hear it’s terrifying. Dr. Bishop did it w such care minimal pain. Definitely recommend this entire facility.
