Meniscal tear

The meniscus is a C-shaped cartilage ring on the inside and outside of the knee joint. This structure is important because it acts as a shock absorber in the knee. The meniscus may be torn at any age, however the risk of tearing increases as we age.

Symptoms

Symptoms of meniscal tear include mechanical joint pain. Mechanical joint pain is the feeling that something is shifting or painful inside the knee with specific twisting or bending of the knee joint. Often it will localize to one side of the knee over the other. In some cases, symptoms can include locking of the knee. In these instances, care should be sought immediately. Commonly, tears have an abrupt onset and are associated with an injury.

Treatment Options

Diagnosis is confirmed with an MRI scan after routine X-ray and physical exam evaluation. 

Treatment of the tear depends on the type of tear and the health of the knee joint. For younger healthier patients with no significant arthritis in their knees, surgery is usually recommended. In patients who have more advanced arthritis, a more conservative approach may be taken since a successful surgery is less predictable. Surgical decision-making in patients with advancing arthritis is highly individualized and based on a more holistic view of the patient’s situation.

Bursitis of knee

Bursa sacs are common structures found throughout the body in areas of friction where structures must slide over each other. In the case of the knee, there is a bursa sac that sits under the skin and over the front of the kneecap. Most often this becomes inflamed after patients are working on their hands and knees on a hard surface. It often presents as a fluid filled bubble over the front of the knee.

Treatment Options

Diagnosis is confirmed with routine physical exam and X-rays.  Treatment for bursitis includes conservative management using ice, compression, anti-inflammatory medications, and draining of the bursa with steroid injection.

Deformity of the knee

Deformity of the knee can occur for various reasons. These can either be conditions that you were born with or developed during childhood, or they can be acquired due to degenerative change in your knee joint or fracture. Deformity can increase joint pressures on one side of the knee and can result in early arthritis.

Treatment Options

Diagnosis is made with routine evaluation and X-rays.  Treatment may include symptom management using medications, injections, bracing, and physical therapy. Surgery may be indicated to prevent further progression of arthritis. Total knee replacement is often recommended in degenerative cases when non-surgical management has failed.

Gout and Pseudogout of the knee

out and pseudogout are known as crystalline arthropathy. These conditions arise when certain chemicals become too highly concentrated in joints and turn into crystals. These crystals cause severe inflammation inside of joints and can become embedded in the cartilage tissue itself causing early degradation of cartilage. Gout almost always begins in the big toe and previous history of gout in that location is common. Pseudogout is more commonly found in the knee. 

Diagnosis

Diagnosis of gout is made partly through history and physical but also confirmed with laboratory studies to check uric acid levels. Pseudogout is often diagnosed by X-ray when characteristic calcifications are noted in the cartilage structures.

Treatment Options

Treatment is similar to other forms of arthritis and aimed at managing joint symptoms. Gout may be treated with an oral medication known as allopurinol. An acute flare up of gout can be managed with colchicine and non-steroidal anti-inflammatory medications. These conditions are often treated by a primary care provider until symptoms lead to arthritic wear and surgical treatment is needed. 

Knee Sprain

Ligaments are thick structures that connect bones around joints.  There are four major ligaments around the knee ACL, PCL, LCL, and MCL.  A knee sprain refers to stretching or tearing ligaments in the knee. When this happens, pain and instability are usual symptoms. Sometimes the knee will swell dramatically due to internal bleeding of the joint.

Treatment Options

The diagnosis is made with routine X-rays and physical exam and confirmed with an MRI scan.  Treatment may be conservative or surgical depending on which ligament is torn as well as other patient factors.

Dr. Sanders, from Cascade Orthopedic Specialists, did a much needed right kbee replacement for me in October 31, 2024. I received top level care from their staff, both pre-op and post-op. Hospital care and treatment at Mercy was excellent. Rehab at Theropy Plus was painful for the first two weeks, but healing came quickly. I now have great range of motion and living pain free with my new knee. Thank you, everyone.

Was referred to Cascade to see Dr. Bishop . For my plantar fasciitis.From the scheduling of appointments, front desk help Amanda and my initial appointment w Dr. Bishop hands down the best experience ever. Had my 2nd appointment for cortisone shot, from the stories you hear it’s terrifying. Dr. Bishop did it w such care minimal pain. Definitely recommend this entire facility.
